Make a Referral to Creative Living Care
Whilst we can accommodate referrals on an emergency basis, we are committed in supporting young people to the best of our ability. As such, we offer a planned transitional service that promotes engagement with and from young people, who are introduced to staff and the service prior to their move. Young people will be assessed prior to placement to ensure it is right for them and to identify the level of support they need to enable them to make a successful transition to independent living.
We do consider both short and long term placements for young people.
